Welcome to Self-Determination Housing of Pennsylvania’s Information & Referral Request page. Please keep in mind that SDHP does not provide direct services to individuals.  Our resources are available to service providers requesting assistance in Pennsylvania.  Please fill out the below form, and a Regional Housing Coordinator (RHC) will contact you within 3 business days.

Information & Referral Request

SDHP staff are available to assist service providers as they navigate housing with their clients. Service providers are encouraged to reach out to their local RHCs for support and guidance related to things such as: searching for housing, landlord relations, etc. This link can also be used to request a private training for your organization or community. An Information & Referral Request can be submitted here (link opens in a new window)-- please be as detailed as possible.
